    Summary:


    Under the general supervision of the Director Pharmacy Services, the Clinical Pharmacist formulates and dispenses all prescribed pharmaceuticals for prenatal through geriatric patients and employees of the hospital.

    Additionally, the incumbent provides guidance and work verification to Pharmacy Technicians. The incumbent performs all duties and tasks in accordance with performance standards established for the job. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position.

    The incumbent may have access to highly confidential patient, employee and/or Organizational proprietary information, and must handle and protect the information in accordance with hospital and system policies, HIPAA requirements and the highest level of ethical standards.

    The incumbent is responsible for reporting all security events, potential events, or other security risks to the organization.

    The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position, and conducts all job responsibilities according to the Mission and Values of the Hospital.


    Responsibilities:

    • Interprets all physician orders for pharmaceuticals.
    • Screens for clinical appropriateness (drug, dose, route, etc.).
    • Screens for allergies and interactions.
    • Maintains accurate patient profile.
    • Communicates with prescribers as necessary.
    • Monitors medication use.
    • Calculates creatinine clearance and verifies dosing as necessary.
    • Performs pharmacokinetic dosing when requested and intervenes when appropriate.
    • When requested by practitioner, autonomously manages the use of aminoglycosides and vancomycin and other pharmaceuticals. Orders dose changes and laboratory studies where appropriate.
    • Performs drug usage evaluations as part of CQI.
    • Communicates with prescribers as necessary.
    • Monitors the use of parenteral nutrition and when requested by practitioner, autonomously manages the use of parenteral nutrition in patient including the review of the patient charts and laboratory values. Orders laboratory studies and formulation changes when appropriate.
    • Attends multidisciplinary team rounds and provides input when appropriate.
    • Reviews orders or chemotherapeutics for appropriateness and intervenes when appropriate. When requested by practitioner, autonomously doses chemotherapeutic agents.
    • Monitors and reports adverse drug reactions to appropriate individuals/agencies.
    • Is cognizant of the cost of drug therapy and intervenes to provide pharmacoeconomic input when appropriate.
    • Prepares compounds and dispenses medications.
    • Ensures that medications dispensed correspond to written orders.
    • Employs compounding and aseptic techniques consistent with provision of high-quality pharmaceuticals.
    • Performs checks of all medications prior to dispensing.
    • Supervises/trains technicians and pharmacy students. Acts as a resource in day-to-day operations.
    • Trains new technicians/students.
    • Trains all technicians/students in new procedures.
    • Checks all technician/student work for accuracy prior to dispensing.
    • Ensures that technicians/students adhere to departmental policies and procedures.
    • Maintains controlled drug records.
    • Ensures that records meet DEA requirements.
    • Performs routine inventories.
    • Audits Nursing and Pharmacy records for accuracy and completeness; intervenes as necessary.
    • Assists in inventory control
    • Provides input to Inventory Control Technician for re-orders.
    • Provides special order information to Inventory Control Technician.
    • Authorizes borrow/loan transactions.
    • Maintains departmental policies and procedures.
    • Provides input to Director Pharmacy Services to keep policies and procedures in line with practice.
    • Helps to develop new policies and procedures as necessary.
    • Provides drug and medical information to physicians, nurses, and other healthcare professionals when requested or as appropriate.
    • Utilizes resources such as medical journals, texts, and the Internet to provide evidenced-based information.
    • Performs additional duties as required or assigned.
    • Prepares pharmacy newsletter.
    • Serves on various hospital committees in which pharmacist participation is needed.
    • Provides inservice educational programs to hospital staff when needed
