    In collaboration with the Stroke Program Medical Director, oversees and works to ensure that the Stroke Program meets the requirements of the certifying body and the communities we serve.

Our caregivers are comprised of highly trained nurse stroke coordinators, neuro hospitalists, fellow ship-trained neuro intensivists, neurosurgeons, neuro-interventional surgeons, and 24/7 fellowship-trained neuro-radiologists. Intermountain Health's front-range comprehensive stroke center supports 3 other in-network primary stroke centers as well as the region at large.

    • Evaluate and analyze Stroke Alert and other pertinent Stroke data for performance improvement activity planning and evaluation within the stroke program.
    • As applicable to the care site program, the Stroke Coordinator or designee identifies and maintains an active list of stroke patients within the facility and supports patient care rounds in collaboration with the Stroke Program Medical Director and clinical team members. Supports stroke education to patients and family members during the hospital stay. Collaborates with Care Management for establishing needs at discharge and serves as a resource and contact for patients and their families after they are discharged.
    • Facilitates linkages between stroke survivors and caregivers with local Stroke Support services.
    • Discusses plan of care with clinical team members and concurrently reviews that all care meets American Heart Association (AHA)/American Stroke Association (ASA) evidence-based guidelines for stroke.
    • Serves as a liaison for the Stroke Program with other departments in the hospital and with outside agencies and institutions. Actively plans and participates in Stroke Education programs for physicians, nurses, and ancillary personnel in the hospital, community, and outlying areas.
    • Collaborates with Stroke Coordinators at other Intermountain Healthcare facilities to share best practices and performance improvement initiatives.
    • Serves as a member of the Acute Stroke Team and responds to stroke alerts as possible to evaluate stroke protocol and care, and provide supportive education to team members as needed.
    • In collaboration with the clinical team, develop and implement evidence-based policies and standards for the Stroke Program by analyzing current guidelines and processes and per DNV/TJC requirements.
    • Promotes the mission, vision, and values of Intermountain Healthcare, and abides by service behavior standards.
    • Performs other duties as assigned.
    Minimum Qualifications

    Required
    An Associate's Degree is required. Annual stroke-specific education hours are required as dictated by the certifying body.
    Current license as a Registered Nurse in the state of practice is required Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP)certification upon hire or obtained within 60 days of hire with prior approval (ACLS/NRP/PALS as appropriate for unit or service ) Clinical certification, license or registration per clinical educational background is required for TJC programs. RN is required for DNV programs. NIHSS and BLS are required
    Three years of relevant RN experience is required.

    Preferred
    A bachelor's degree is preferred.

    A current State RN License is preferred. ACLS preferred.
    Certification as appropriate for unit or service is preferred

    The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
