Job description
Under the direction of the CEO, the Licensed Clinical Social Worker (LCSW) will provide direct services tosenior adults and caregivers from a variety of referral sources. <\/span>
<\/div>
<\/div>
This position connects patients and
community members to program offerings and community resources and may also serve as a
representative. <\/span>
<\/div>
<\/div>
The LCSW will also work in conjunction with the patient's family, caregiver, nurse, and
clinician to provide optimal support services. <\/span>
<\/div>
<\/div>
This position will help the patient gain the strength to
carry on with daily like skills, improve quality of life and help them remain independent.<\/span>
<\/span><\/div>

Requirements<\/h3>
Provide mental or clinical social work services both telephonic and in the home of clients <\/span>
<\/div>
<\/div>
Carry out comprehensive social work assessments by phone or in -person via home visits to
develop a plan of care for the client <\/span>
<\/div>
<\/div>
Work directly with patients and their families to address unmet needs and provide the
necessary social work services to help patients and support achieve their care goals <\/span>
<\/div>
<\/div>
Maintain relationships with referring partners through consistent and timely communication <\/span>
<\/div>
<\/div>
Promote the STARS program and the STARS CRDC Resource Centers program offerings by
helping clients to connect with programs that address their needs <\/span>
<\/div>
<\/div>
Evaluate patients (existing, new, and prospective) based on their needs, limitations, and desires <\/span>
<\/div>
<\/div>
Address concerns and goals while maintaining constant communication with the patient <\/span>
<\/div>
<\/div>
Build rapport with patients and their families <\/span>
<\/div>
<\/div>
Coordinate care with PT and OT <\/span>
<\/div>
<\/div>
Respond to crisis situations that affect vulnerable or mentally unstable seniors <\/span>
<\/div>
<\/div>
Advocate and raise awareness on behalf of clients and the needs of the community<\/span>
<\/div>
<\/div>
<\/div>
Required Qualifications:
<\/span>
<\/div>
<\/div>
Master's degree in social work, psychology, sociology, healthcare, or another related field <\/span>
<\/div>
<\/div>
Valid and current LCSW (Illinois) <\/span>
<\/div>
<\/div>
2 -5 years of professional social work experience <\/span>
<\/div>
<\/div>
Experience in Case Management <\/span>
<\/div>
<\/div>
Working with geriatric patients<\/span>
<\/div>
<\/div>
<\/div>
Preferred Qualifications:
<\/span>
<\/div>
<\/div>
Master's in Social Work <\/span>
<\/div>
<\/div>
Home Care Hospice experience <\/span>
<\/div>
<\/div>
Bilingual Spanish<\/span>
<\/div>
<\/div>
<\/div>
