Job Summary:

Responsible for collaborating with the health care team to coordinate cost-effective services and enhance quality of life for a defined population.

Essential Responsibilities:

• Problem identification, psychosocial assessment, and accessing community resources.
• Develop and monitor care plans to enhance quality of life.
• Advocate for organizational and community resources.
• Ensure documentation compliance and analyze outcomes.
• Facilitate accessing funded programs and provide counseling.
• Promote self-awareness and maintain social work standards.

Basic Qualifications:
Experience

• Minimum 3 years clinical experience plus 2 years in case management.
• Minimum 1 year experience with the defined population.

Education

• Masters degree in social work (MSW) required.

License, Certification, Registration

• Credentials from primary work state required before hire.


• Licensed Clinical Social Worker (Virginia) within 6 months of hire.


• Licensed Clinical Social Worker - Certified (Maryland) within 6 months of hire.


• Independent Clinical Social Worker License (District of Columbia) within 6 months of hire.


• Case Manager Certificate within 36 months of hire.

Preferred Qualifications:

• Experience with computer software programs in a Windows environment.
• Knowledge of community systems and resources in the service area.
• Knowledge of regulatory issues for the Mid-Atlantic area.
• Clinical social worker experience.
• MSW required.
