    OverviewWe at Fairview Health Services are looking for a dedicated Casual Pre-Licensed Mental Health and Substance Use Disorder Psychotherapist We deliver comprehensive treatment modalities for adults struggling with mental health and substance use concerns.

    Facilitates treatment planning and leads each case. Responsible for development of transition/discharge plan. Communicates optimally and respectfully with others, including interdisciplinary team and all other contacts. Functions as an interdisciplinary team member. Accepts consultation with a licensed supervisor for the functions of this position in their clinical practice.

    Within this program, the MICD Psychotherapist Trainee acts as a part of the treatment team providing therapeutic interventions that include individual therapy, family support, group counseling facilitation, case management support, assessment of mental health & substance use, crisis intervention, skill building, and advocacy for patient needs.

    This role will join a multidisciplinary treatment team that includes nurses, psychiatric associates, chaplains, other counseling professionals (pre-licensed counselors, LADCs, LPCCs, LISCWs, LMFTs, & LPs), nurse practitioners, and psychiatrists.

    Facilitating group/individual sessions; case management; 245G and MHealth compliant documentation; coordinate care and collaborate with program psychotherapist. Use technology to facilitate hybrid groups. Use electronic medical record, Epic.
    Responsibilities Job DescriptionFacilitates treatment planning and coordinate management of each case.
    Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team.

    Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

    Implements, reassesses and adjust treatment plan as vital.
    Completes an interdisciplinary assessment tool with each patient.
    Updates interdisciplinary assessments each week or per regulatory requirement.

    Collaborate with government agencies such as court, ombudsmen, county servicesCompletes referral process for next level of careCommunicates to other providers in the communitySets up intake appointmentsCompletes needed paperwork to facilitate this process i.e.

    applications, letters of recommendation, etc.

    Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.

    Implements, reassesses and adjusts treatment plan as vital.
    Communicates frequently and efficiently with interdisciplinary team to ensure continuity and coordination of care.
    Documentation is detailed, timely, legible and clinically appropriate and meets regulatory requirements.
    Demonstrates competence in developing a clinically relevant transition/discharge plan.

    Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge planDevelops discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.

    Documents thoroughly, timely, legible and clinically appropriate.
    Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups. Provides individual, couples and family therapy.
    Demonstrates the ability to advance evidence-based practice by using standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are improved.

    Promotes a culture of evidence-based practice by participation in any of, but not limited to, the following activities:QualificationsMaster's degree in behavioral field working toward mental health licensureWorking towards independent licensure: LPCC, LICSW, LMFT, & LP.Holds a current and active LADC.

    12 hours of co-occurring educationHas completed a minimum of 30 hours course work or one 3-credit semester college course in adolescent developmentHas 150 hours of supervised experience as an adolescent counselor either as a student or staff memberBasic Life Support certificate Preferred:2 or more years of experience in mental health and chemical dependency
