    Overview of the Position:

    The Staff Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient's health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support. This position will be located at various locations as assigned, require some 12 hour shifts and rotating Saturdays.

    Education and Experience:


    • Diploma in Nursing; minimum requirements or Associates in Nursing


    • Licensed by the State Board of Nursing


    • BLS required


    • Two year medical office experience preferred

    Required Skills / Core Competencies:


    • Must possess a current driver's license


    • Must have dependable, reliable transportation required to commute to other locations within the organization, as needed.


    • Reliable mode of communication (mobile or home telephone) with messaging capability


    • Must possess excellent oral and written communication


    • Must have the ability to collaborate with others to improve the patient experience and address needs


    • Works with Patients, Families and Providers


    • Must possess excellent oral and written communication

    • Maintain current general medical knowledge


    • Ability to take initiative and handle various task simultaneously


    • Knowledge of policies, procedures, regulations, and protocols


    • Knowledge of data bases (ex: EMR systems)


    • Bilingual in English/Spanish preferred


    • Familiar with medication names and usage


    • Medical terminology


    • Excellent interpersonal skills

    • Ability to multitask


    • Compassion for patients


    • Ability to administer a variety of treatments and medications as directed


    • Capable of adapting to computer software

    Additional Requirements:

    As a participant in the Staffing Pool, the Staff Nurse will be available upon requested by HopeHealth, Inc. to work at any given location on the basis of staffing needs. As such must adhere to the same employment requirements as other staff of HopeHealth, Inc.


    • As a participant in the staffing pool, the Staff Nurse – Float will work where assigned and communicate any difficulties with their direct supervisor or designee.


    • The Staff Nurse – Float is compensated at the agreed upon hourly rate; plus mileage if applicable. Travel is paid based on the HopeHealth reimbursement policy and assigned base location for mileage purposes. (All mileage is due to your direct supervisor by end of business on payroll Friday to ensure required approval signatures.)


    • The Staff Nurse – Float is responsible for travel to the locations as assigned; therefore, must have reliable transportation to remain in the staffing pool.


    • The Staff Nurse – Float must be available to work between Monday – Friday from 7:30 am to 6:00; you may also be asked to work Access Hope which hours are Monday – Saturday from 8 am to 8 pm. Your hours will be adjusted to if you are scheduled to work an Access Hope shift. Access hope shifts will rotate amongst the Floater Staffing Pool as appropriate.


    • HopeHealth, Inc. reserves the right to alter and/or cancel this Staffing Pool at any time in its sole and absolute discretion.


    • It is the responsibility of the Staff Nurse – Float to keep contact information updated with supervisor at all times.

    Supervisory Responsibilities:


    • This position has no direct reports

    Essential Job Functions:

    These functions will be performed by the Patient Support Representative and will be the responsibility of the incumbent working with the patients, medical providers, clinical staff and non-clinical staff to ensure excellence in service for the patient.


    • Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data


    • Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.


    • Assist patients and providers in obtaining medicinal prior authorizations when required


    • Verifies the patient understands the purpose of the visit.


    • Providers preventative Care information matching the patients age, gender, and health status


    • Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment


    • Knowledge of examination, diagnostic and treatment room procedures


    • Ability to interpret, adapt, and apply guidelines and procedures


    • Basic Critical Care and Chronic Illness Management


    • Required to participate in training opportunities


    • Helps promote office efficiency and encounter volumes


    • Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients


    • Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs


    • Willingness to learn about AIDS Service Organizations and Community Health Centers


    • Assist in training new nurses to the organization in the standard working procedures of HopeHealth


    • All other duties assigned

    Physical Requirements:

    Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
