    Position Summary

    The Salvation Army Northern Division is looking to hire a Case Manager - Pathway of Hope (POH) position. The Case Manager - Pathway of Hope (POH) is responsible for providing targeted services to families with a desire to break the cycle of crisis and enable a path out of intergenerational poverty. The Case Manager recruits and provides a full range of case management services.

    Essential Functions (approximate time spent)

    • Provide long-term strength-based, client-centered, holistic case management to POH families (75%).
    • Maintain up-to-date, accurate client information in casefiles and through database entry (10%).
    • Recruit qualifying families for POH and promote social services and corps programs with clients and throughout the community (5%).
    • Engage in internal activities to promote and further POH goals and objectives (5%).
    • Facilitate individual and group educational activities (5%).

    Education and Experience

    • Bachelor's degree in relevant field required.
    • If degree is in social work, must be licensed by state board.
    • Previous social services or ministry experience required.
    • Experience conducting long-term case management preferred.
    • Must pass background checks.
    • Valid driver's license and meet MVR requirements, preferred.
