- Provide long-term strength-based, client-centered, holistic case management to POH families (75%).
- Maintain up-to-date, accurate client information in casefiles and through database entry (10%).
- Recruit qualifying families for POH and promote social services and corps programs with clients and throughout the community (5%).
- Engage in internal activities to promote and further POH goals and objectives (5%).
- Facilitate individual and group educational activities (5%).
- Bachelor's degree in relevant field required.
- If degree is in social work, must be licensed by state board.
- Previous social services or ministry experience required.
- Experience conducting long-term case management preferred.
- Must pass background checks.
- Valid driver's license and meet MVR requirements, preferred.
- Willing to support and promote the philosophy, goals, and mission of The Salvation Army.
- Flexible interpersonal and communication skills.
- Excellent organizational skills and attention to detail.

Position Summary

The Case Manager - Pathway of Hope (POH) is responsible for providing targeted services to families with a desire to break the cycle of crisis and enable a path out of intergenerational poverty.
