Position: Case Manager
Location: Morristown, NJ
Pay Rate: $ 57/hr
Schedule: 8am - 4pm
Duties:
-Coordinates patients through the acute care hospitalization
-Manages all aspects of discharge planning/transition process
-Performs utilization management and quality screenings
-In- Patient Case Management is required
Skills:
2 years of acute care experience strongly preferred
