Role Description:
Here at Oak Street Health, we're looking for a Primary Care Physician to provide top-notch healthcare to our local medicare patients in our community-based primary care centers. Our Physicians work with dedicated teams, focusing on quality care rather than quantity.
We reward our providers through a care model that values outcomes, reduces hospital admissions, promotes preventive medicine, and ensures an exceptional patient journey. If you're passionate about offering excellent care to older adults, including fresh graduates, we want to hear from you.
Outstanding candidates may also be eligible for the role of Center Medical Director.
Responsibilities:
- Assess and diagnose patients in our clinics
- Oversee primary care operations
- Prescribe medications
- Maintain patient records using canopy and greenway
- Collaborate to achieve healthcare quality goals
- Manage the care team
- Other duties as needed
Required Qualifications:
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)
- Board Certification in Internal Medicine or Family Medicine
- State License and DEA License
- US work authorization
- Alignment with Oaky values
Preferred Qualifications:
- Geriatrics Fellowship training
- Experience with Geriatric populations
- Proficiency in Value-Based Healthcare Model
- Previous work in outpatient primary care
- Bilingual proficiency is a plus
