    We are currently seeking a compassionate RN Case Manager to make a meaningful impact by facilitating life-saving care

    Role Overview:

    As an RN Case Manager, you will play a pivotal role in coordinating care, managing transitions, and planning discharges while optimizing utilization across the acute care spectrum. Collaborating closely with physicians, medical social workers, and bedside RNs, you will ensure patients receive timely, appropriate care, minimizing unnecessary admissions or readmissions. Your responsibilities extend to overseeing acute patient populations across the care continuum, delivering integrated and timely care. Reporting to the Supervisor, Manager, or Director of Care Coordination, you will engage frequently with patients, families, interdisciplinary teams, and various stakeholders to ensure optimal patient outcomes.

    Requirements:

    • Education: Bachelor's degree in Nursing or Health Administration. Desired: Master's degree in Nursing, Case Management, or related field.
    • Certification & Licensure: Registered Nurse (RN) in California. Certified Case Manager (CCM) preferred. Basic Life Support (BLS) Healthcare Provider certification required.
    • Experience: Minimum of three (3) years in acute medical/surgical/ED or critical care nursing. Consideration for employment with a Master's in Nursing in Case Management instead of three (3) years of acute care experience may be at the director's discretion. Previous Case Management experience preferred. Experience with electronic InteQrual or other standardized criteria strongly preferred.
