About the Position:

During a visit lasting up to one hour, Matrix providers assess a member's health, medical history, medication adherence, social environment, and other risks. This comprehensive Health Assessment provides valuable insights into a member's well-being that may be missed during typical office visits, leading to improved care quality and the potential to address multiple care gaps in a single visit.

Responsibilities:

Matrix Provider – What to Expect:

• Perform Adult/Geriatric Assessments, diagnose and treat medical conditions, provide health education, make physician referrals, manage cases, and maintain clear documentation following Matrix guidelines
• Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical staff
• Work in various settings including home visits, skilled facilities, and telehealth appointments

Nurse Practitioner - Part Time/PRN - Home Risk Assessments

Type: Part Time/PRN - Flexible-Nurse Practitioner

PAY: $105.00 or $110.00 per completed Assessment (based on county)

Location: Home, SNF, Telehealth, and other community environments in Florence, South Carolina

Hours: Flexible schedule including days, evenings, nights, and weekends

Benefits: Non-benefited for PRN/Part Time roles, except for 401(k) and limited Voluntary Benefits

Mileage reimbursement plan available

Qualifications:

Must Haves:

• Master's Degree required OR relevant experience with NP licensure
• Current RN and NP licensure with prescription authority or eligibility to obtain it
• Board certification in a Matrix approved specialty
• Valid BLS, ACLS, or CPR certification
• Minimum 1 year experience as a Nurse Practitioner
• Proficiency in computer skills and medical record software
• Excellent verbal and written communication skills
• Adaptability to change and flexibility

Travel Requirements:

• Ability to travel with a valid driver's license and proper insurance coverage
• Travel may be required to reach patients at their residences
