    General Position Description:

    Under general supervision provides guardianship support services to wards appointed to Phoenix Counseling Center (PCC) by the local Clerk's of Court. Primary duties include working with the wards to obtain housing, attending medical appointments, person-centered planning meetings, providing transportation, conducting visits with the wards and significant others, completing documentation and maintaining guardianship files, assisting in times of crisis, and other duties as assigned by the supervisor.

    Performance Expectations (Quality and Quantity):

    Shall demonstrate the use of knowledge/skill/judgment and provide guardianship support services that evidence quality, and performance, and meet the needs of wards served and other stakeholders. Perform tasks shall include, but not be limited to:

    · Staff shall be knowledgeable of housing resources

    · Able to work with residential providers across the network· Able to assist with placement and follow through

    · Understand the financial eligibility systems, (Medicaid, IPRS, Social Security, Supplemental Security Income) and ability to assist wards

    with applying for and obtaining such.

    · Knowledge of requirements for all documentation both legal and contract requirements. Will assist with maintenance of ward's records.

    · Knowledge and ability to assist with court hearings and the ability to represent PCC

    · Understanding the person-centered process and ability to attend meetings to assist and advocate for the wards.

    · Knowledge of the MH/SA/IDD service system, and services provided to assist with referrals and follow-through.

    · Ability to assist in times of crisis and knowledge about crisis systems and supports.

    · Transport wards to and from appointments, meetings, residential placements, community resources, etc.

    · Ward record documentation to include:

    § Contact notes

    § Confidentiality and Privacy Practice documentation

    § General admission and referral forms

    · Assist guardianship specialists in times of crisis with wards and obtaining needed services

    · Ability to effectively communicate with wards to understand the guardianship process

    · Coordinate assignment of tasks with the guardianship specialist

    · Use a community-based team approach that utilizes the resources of the community consumer, and treatment team to achieve positive

    outcomes, stabilize crisis' and appropriate transition of the ward to needed services and supports.

    · Shall demonstrate the ability to work/be a part of an organizational and therapeutic team, and maintain positive working relationships

    with wards, significant others, providers of services, community stakeholders, referral organizations, and other employees/departments

    of PCC.

    · Shall develop a working knowledge of applicable organization policies and procedures and demonstrate compliance. This shall include,

    but not be limited to:

    § Personnel policies/procedures

    § Employee Code of Ethics/Corporate Compliance/Consumer Rights

    § Service record documentation

    § Confidentiality/HIPAA

    § Clinical policies/procedures

    · Shall complete other duties as assigned by the supervisor

    General Knowledge and Skills (will possess at time of employment or will be expected to learn upon employment): This position requires a comprehensive knowledge of laws, regulations, and policies that pertain to guardianship; a comprehensive understanding of behavior management techniques and of a wide range of family and group dynamics and a wide range of intervention techniques; of governmental and private organizations and resources in the community; excellent judgment and initiative; absolute discretion; demonstrated ability to work under pressure and meet deadlines; demonstrated ability to work with significant independence of action; demonstrated ability to complete documentation timely and with accuracy; to complete and file clear and factual reports.

    Preferred and Minimum Position Qualifications:

    · High school diploma or GED

    · Minimum of 2 years experience with the population in areas of MH/SA/IDD service provision

    · Preference will be given to individuals who have a valid Peer Support Specialist certification

    Additional Job Requirements (License, Certifications, Personal Vehicle, etc):

    · Valid driver's license

    · Travel is required as a part of this position and you will have access to a company vehicle.

    · Must have the ability to operate a telephone, fax machine, and computer hardware/software.

    · Must have the ability to write in a manner that is legible/readable/understandable.

    · Willingness to work in a high-stress/risk environment that is driven by team performance

    · Transportation to and from work in various environmental conditions is essential
