- Provide coordinated formal outreach services to prepare potential clients to accept services and formal outreach services to triage and register clients.
- Provide initial medical/psychological and additional assessments to registered clients leading to the definition of treatment issues, the development of an individualized care plan and the implementation of a firm referral.
- Provide counseling and case coordination services to follow through on referrals and monitor client responses to services. May function as a case manager for clients when necessary.
- Maintain and protect the health and safety of clients and O'Hare occupants within the limits of the position holder's capabilities. Maintain office cleanliness.
- Provide documentation in logs and client records of services delivered. Assist in the coordination of records, building databases on current program files.
- Other duties as required for effective teamwork or as assigned by O'Hare Outreach Supervisor
- High School Diploma plus one year of experience, preferably with the homeless or mental healthpopulation.
- Assessment and Case Management skills, as well as written and verbal communication skills
- Must be able to walk and stand for extended periods of time
- Drivers License required, must be able to drive agency van
- Computer skills are essential
- No felony convictions
