    Shift Details: 8 hour day shifts with rotating weekends

    Summary:

    The Physical Therapist (PT) is a professional who is responsible for assessing, planning, implementing, and evaluating patient care. The PT is ultimately responsible for implementing and documentation of interventions, outcomes, discharge plans and patient/family/care giver education. He/she collaborates with physicians and other members of the interdisciplinary health care team to provide continuity of care. The Physical Therapist is responsible for the supervision of and delegation of tasks to appropriate individuals including Physical Therapist Assistants (PTA), technicians, students, and other customers of the organizations. The PT must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. He/she reports to the Director of Rehabilitation Services.

    1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups:

    ☐ Neonate (birth - 28 days)

    ☒ Infant (29 days - less than 1 year)

    ☒ Pediatric/Child (1 year - 12 years)

    ☒ Adolescent (12 years - 18 years)

    ☒ Adult (18 years - 65 years)

    ☒ Geriatric (over 65 years)

  • Incorporates cultural considerations in the provisions of care.
    * Knowledge of growth and developmental stages.
    * Considers life changes/effects on health beliefs and behaviors.
    * Provides necessary safety measures.
    * Provides information and involves family/caregiver in decision making.

    2. Performs all responsibilities/duties required by the Rehab Therapies Department as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes, but is not limited to, the age of the patient served.

  • Practices Physical Therapy consistent with the American Physical Therapy Association Code of Ethics and Standards of Practice, and the State of Nebraska Statutes, Rules and regulations relating to physical therapy.
    * Stays abreast of new techniques and trends related to Physical Therapy through active professional development plan.
    * Interprets and implement prescription or referrals from physicians.
    * Performs patient evaluations using tests and measurement tools appropriately.
    * Establishes Plan of Care with measurable goals utilizing consultation with appropriate individuals; including but not limited to the patient, family/care giver and significant other, he physician and other health care team professionals.
    * Competent use of physical therapy interventions to assess, prioritize, formulate, implement, evaluate and adapt the interdisciplinary plan of care to meet the patient's age-specific needs.
    * Initiate the discharge plan as appropriate and provide continuity of care to meet the patient's changing needs and achieve the outcomes.
    * Provides timely, articulate and accurate verbal and written communications with physician and other health care team members.
    * Provides documentation, which includes treatment given, patient response to treatment, and is timely, legible, accurate, inclusive and concise.
    * Provides appropriate supervision of PTA's and technicians.
    * Identifies educational needs for patient/family/caregiver.

    Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

    EDUCATION:

  • Graduate of a professional physical therapist education program accredited by the Commission of Accreditation in Physical Therapy Education (CAPTE) required.
    * Bachelor's Degree required.
    * Master's Degree preferred.
    * Doctoral Degree preferred.

    CERTIFICATES, LICENSES, REGISTRATIONS:

    *
  • Current Physical Therapist license from the State of Nebraska required.
    * Basic Life Support (BLS) obtained within one full month from date of hire required.

    Skills/Knowledge Requirements:

  • Language Skills - Ability to read, write, speak, and understand the English language required.
    * Entry Level Computer Skills required.

    Other Certifications/Requirements:

  • Current, valid driver's license issued in the state of legal residence required.
    * Operating FRHS-owned vehicles is required.
