    OVERVIEW

    Boston Senior Home Care is a private, nonprofit organization that provides case management, person-centered care planning, home care and long term services and supports to those who wish to remain in the community but require assistance to do so. One of 26 Aging Services Access Points (ASAPs) in Massachusetts, Boston Senior Home Care is committed to providing the best assistance possible for those in need. Although the majority of the people we assist are over age 60, we offer long term services and supports for children and young adults. We also provide programs and education initiatives for those serving as caregivers and information about maintaining a healthy lifestyle and controlling chronic disease at any age.     SUMMARY OVERVIEW

    Case Managers conduct visits in the Greater Boston area to assess and evaluate elder's eligibility for the State Home Care Program as well as other community resources. Case Managers monitor their assigned client caseload through telephone contact. Case Managers work collaboratively with informal and formal supports, vendors and other community agencies.

    ESSENTIAL FUNCTIONS

    Essential Job functions:Duties 1 through 11 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.

    1. Special Projects as assigned
    2. Provide on call coverage for the department 3x/week
    3. Follow up on Department Reports
    4. Respond to Billing inquiries
    5. Carry a small caseload and conduct visits to assess and evaluate elders to determine eligibility for state home care services and/or make recommendation for community
    6. Utilizing EOEA guidelines assess care plan based on individual unmet needs of the Ensure interdisciplinary review.
    7. Implement the plan-based utilizing BSHC community resources
    8. Conduct visits according to the guidelines of the specific program
    9. Maintain client and care plan monitoring through ongoing vendor communication and phone calls to client and informal Revise care plan as needed
    10. Maintain the client record by the timely documentation of all significant communications from vendor, client and informal
    11. Maintain a working knowledge of BSHC and other community programs or have the skill to access this
    12. Develop and hone skills to advocate for clients in a wide variety of
    13. Contribute to organizational life through active participation in team activities, committees, and work Promote a positive working environment through cooperative and respectful interaction with others.
    14. Consult with Team Managers and other staff as needed for ongoing case management and other
    15. Conduct ongoing outreach efforts in order to ensure access to services for all client

    COMPETENCIES

    • Sensitivity to elder issues and commitment to maintaining clients at home with dignity.
    • Ability to effectively communicate in person and by
    • Computer experience with the ability to perform data entry and word processing.
    • Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather
    POSITION TYPE/EXPECTED HOURS OF WORK

    This is a full-time position, Monday - Friday 9:00AM-5:00PM

    PREFERRED EDUCATION AND EXPERIENCE
    • Bachelor's Degree in social work, human services, nursing, psychology, sociology, or related field
    • SW, LSW and relevant work experience preferred, but not required.
    WORK AUTHORIZATION/SECURITY CLEARANCE
    • Must clear CORI background check
    • Must be able to work in the United States
