    Youth and Family Services are for children and families who need various case management, counseling, crisis, and psychiatric services. The Hampton-Newport News Community Services Board seeks a dynamic Support Coordinator to join our team and play a vital role in coordinating and enhancing services for young individuals in need.

    The Child and Adolescent Support Coordinator ensures the continuity of directly provided and contractual services to agencies and individuals with intellectual and developmental disabilities. Support Coordinators assist individuals with their basic physical, financial, social, and personal needs. Support Coordination services for individuals include developing person-centered plans, service provider linkage, monitoring, advocacy, counseling crisis intervention, daily living support, and enhancing community integration.

    Key Responsibilities:

    • Conduct assessments to identify the unique needs of children and adolescents and develop individualized support plans.
    • Collaborate with families, educators, healthcare professionals, and community organizations to coordinate and enhance services for children and adolescents.
    • Advocate for clients' needs and rights in various settings.
    • Provide ongoing support and guidance to clients and their families, ensuring access to necessary resources.
    • Monitor and evaluate the effectiveness of support plans and make adjustments as needed.
    • Facilitate communication and collaboration among service providers to ensure a holistic approach to care.
    • Maintain accurate and up-to-date documentation of client interactions and progress.

    Requirements: Bachelor's degree in Human Services (Social Work, Psychology, or Counseling) with one (1) year of experience in support coordination services with the assigned population. Valid Drivers License.


    The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
