    Description
    Job Summary:
    The Social Worker conducts comprehensive psychosocial assessments to members to enhance their ability to navigate their health care journey effectively. They utilize assessments, interventions, educational programs, and connections to health plan and community resources. The Social Worker promotes individuality, independence, and patient choice while considering cultural and ethnic differences. Additionally, they aid medical teams in understanding patient beliefs and behaviors. Essential Responsibilities:
    • Utilizes age-appropriate assessment and intervention techniques.
    • Assists members in creating cost-effective health care plans using personal and community resources.
    • Promotes patient-centered, multidisciplinary care addressing personal, psychological, economic, and cultural needs.
    • Maintains up-to-date knowledge of Kaiser Permanente services, community resources, and relevant laws.
    • Completes necessary documentation as per departmental policies.
    • Participates in program development and educational activities to empower members in managing their health care.
    Basic Qualifications:
    Experience
    • Minimum of 1 year of Social Work experience in a relevant setting -OR- completion of the Kaiser Permanente NW Social Worker Internship program.
    • Minimum of 3 additional years of related experience.
    Education
    • Masters degree in social work from an accredited university.
    License, Certification, Registration
    • Credentials from primary work state required before hire, additional credentials needed post hire.
    • Masters Social Worker License (Oregon) within 6 months of hire OR Clinical Social Work Associate Certificate (Oregon) within 6 months of hire OR Licensed Clinical Social Worker (Oregon) within 6 months of hire
    • Licensed Social Worker Associate-Independent Clinical (Washington) within 6 months of hire OR Certified Social Worker Associate License (Washington) within 6 months of hire OR Licensed Advanced Social Worker License (Washington) within 6 months of hire OR Licensed Independent Clinical Social Worker (Washington) within 6 months of hire
    • Basic Life Support certification within 3 months of hire.
    • National Provider Identifier required at hire.
    Additional Requirements:
    • Ability to collaborate effectively in a multidisciplinary team.
    Preferred Qualifications:
    • Minimum of 3 years of professional Social Work experience in a relevant setting.
    • LCSW certification.
    • Familiarity with KPNW services and benefits.
    • Proficiency in short-term counseling techniques.
    • Demonstrated leadership skills in a multidisciplinary environment.
    • Advanced knowledge of health care-related resources.
