- EDUCATION/CERTIFICATION/
- Certification in Case Management (CCM, ACM) preferred
- Current RN Licensure in New York; Associates Degree in Nursing, Bachelor's Degree in Nursing (BSN) preferred

This position works for Crouse Medical Practice, a multi-specialty physician practice with several locations to serve healthcare needs in the Syracuse, NY communityJOB SUMMARY:
The Senior Population Health Nurse will serve as a clinical leader in the Population Health and Quality Improvement department, mentoring colleague nurses in assessment and intervention techniques, providing guidance on complex or difficult cases, advising leadership as new clinical programming is developed, and representing the department on improvement teams.#
The Senior Population Health Nurse will also function as a nurse care coordinator and coordinate all systems/services required for an organized, multidisciplinary, patient centered care team approach, and assure quality, cost- effective care for the identified patient population.
#Manage the course of treatment for patients by coordinating care with physicians, nurses, other staff and community health care providers, ensuring quality patient outcomes are achieved within established time frames and with efficient utilization of resources.
S/he interfaces with community providers, insurance companies and agencies and providers along the care continuum.# Direct the work of LPN Care Coordinators.# Participate in Patient Centered Medical Home activities.#This is a remote position.
