    CLASSIFICATION/STATUS: Non-Exempt, Full Time, Permanent

    PAY RANGE: Band 2 ($20-$25/hour)

The Portuguese Community Health Worker will serve as a frontline public health worker and medical interpreter. As part of a dynamic team at Charles River Community Health (CRCH), a healthcare practice providing compassionate care to underserved communities, the Portuguese Community Health Worker will serve as a frontline public health worker and interpreter.

    The Portuguese CHW is a trusted member of the community and has a strong understanding of the Portuguese community. The Portuguese CHW serves as a liaison between the health provider and social services and the community/patients. As such, the Portuguese CHW will facilitate access to services and improve the quality and cultural competence of service delivery. If you have a calming presence, excel at understanding patient needs, and are willing to work on educating patients about the importance of health screenings, we would like to have you as part of our team. The Portuguese CHW will work as a team member at our health center and will interact with patients and staff who are racially, ethnically, and culturally diverse, so being culturally competent is required.

    YOUR RESPONSIBILITIES

    • Conducts accurate medical and general interpretation for patient care staff, patients, and their families.
    • Translate documentation, posters, and flyers.
    • Establishes trusting relationships with patients and their families while providing general support and encouragement.
    • Provides ongoing follow up, basic motivational interviewing and goal setting with patients/families.
    • Provides referral for services to community agencies as appropriate.
    • Remains knowledgeable about community resources appropriate to needs of patient/family.
    • Enables and improves culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
    • Conducts outreach, implements programs in community that promotes, maintains and improves community health needs and provides resources.
    • Collects data to help identify community health needs.
    • Advises community groups on chronic disease and ways of prevention
    • Provides informal counseling as needed.

    YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

    • Bilingual speaking required. Must be proficient in English and a secondary language, Portuguese.
    • Minimum requirement: Highschool diploma or GED.
    • Must have proof of completing a minimum of 40 hours of medical interpretation training. Or willing to obtain training on the job and obtain certification within 1 year of employment.
    • At least 1 year of experience working within a healthcare setting
    • Certification in Community Health Worker or Medical Interpreter preferred but not required.
    • Must have knowledge of computer systems such as Word and Excel.
    • Strong interpersonal, organizational, and time management skills required.
    • Must have a willingness to work flexible hours to meet the organization's needs/demands. Flexible to work on evening and/or Saturdays -8:30a-1:00p (otherwise, there is some flexibility with the schedule).
    • Must be able to work in either site or between sites (Brighton/Waltham) as needed.
    • Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
    • Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
    • Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
    • Knowledge of both community and healthcare system resources.
    • Experience serving the population(s) that will be referred for patient navigation services.
    • Excellent written and oral communication skills.
    • Experience in diverse settings preferred.
