    Job Summary

    Works within the community offering support to patients with complex medical and social needs. Acts as a valuable resource to help patients navigate the healthcare system.

    Job Responsibilities

    • Orients and assesses new Community Health Workers.
    • Conducts regular assessments to determine staff proficiency levels during the orientation phase for effective training decisions.
    • Aids in identifying and reaching out to patients with complex medical conditions.
    • Collaborates with a diverse team to create and implement individualized care plans.
    • Engages with patients through various means like phone calls, home visits, hospital visits, and accompanying them to appointments with healthcare providers.
    • Administers vital signs and provides disease-specific education.
    • Identifies care gaps and connects patients with necessary resources.
    • Facilitates telemedicine visits between patients and healthcare providers.
    • Ensures medication adherence through medication reviews.
    • Carries out safety assessments in patients' homes.
    • Supports adherence to treatment plans by promptly addressing and resolving care gaps.
    • Communicates any concerns to the clinical team and manager, seeking proactive support and guidance.
    • Participates in team conferences to optimize treatment plans.
    • Reinforces treatment plans under the direction of the Case Manager and Licensed Social Worker, offering consultation and case reviews as needed.

    Education

    High School Diploma or Equivalent (GED) - (Required)

    Experience

    Minimum of 1 year-Related work experience (Required)

    Certifications and Licenses

    Valid Driver's License - Default Issuing Body; Certified Phlebotomy Technician - American Society for Clinical Pathology (ASCP)
