    Completing accurate assessments, MDS & care plans as assignedInitiating care plans and supporting activities as assignedCreating and distributing monthly care plan calendars in a timely fashionMaintaining & updating all care plans and assessments as requiredMonitoring & auditing clinical records, ensuring accuracy & timelinessInforming DON of persistent issues related to non-compliant documentationProtecting the confidentiality of Resident & Facility information at all timesREQUIREMENTS:
    MUST HAVE PRIOR MDS 3.

    0 EXPERIENCEValid New York RN LicenseLong Term Care Experience RequiredMust be highly organized, professional & motivatedShould have solid computer skillsExcellent communication skillsShould be friendly and a team workerAbout us:
