Overview: Seeking a compassionate and detail-oriented Hospice Nurse to provide end of life care, ensuring comfort and quality of life for patients. The role requires teamwork, excellent communication with patients and families, and a dedication to celebrating patients' life journeys.
Hours: 9am - 7:30pm, AND every other weekend.
Responsibilities:
Evaluate patients for hospice services.
Inform patients and families about the organization's care options.
Coordinate care plans with physicians and liaise with health professionals.
Market hospice concept through education to professionals and the community.
