    The Role of the Palliative Care Nurse Practitioner

    We are seeking a skilled Palliative Care Nurse Practitioner to join our team. In this role, you will work closely with the Palliative Care Medical Director and other members of the palliative team to deliver comprehensive, individualized palliative care to patients referred to our program.

    Key Responsibilities

    • Assess patients' needs and develop a personalized palliative plan of care in consultation with the patient, family, and other members of the palliative care team.
    • Provide diagnosis, treatment, and follow-up care to patients, including prescribing medications, ordering laboratory and diagnostic tests, and communicating clinical findings to the Palliative Medical Director as needed.
    • Collaborate with the supportive care team, including social workers and spiritual care coordinators, to ensure a holistic approach to patient care.
    • Maintain effective working relationships with healthcare professionals and involve them in patient care plans as necessary.
    • Communicate and document clinical findings, treatment plans, and care provided in the patient's medical record.

    Requirements

    To be successful in this role, you should have:

    • A Master's degree in Nursing from an accredited school of nursing or equivalent.
    • A minimum of two years of experience as a Nurse Practitioner, preferably in Family or Adult Medicine.
    • Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
    • Certification/license as a Nurse Practitioner.
    • A collaborative agreement with a supervising physician agreeable to supervise work in palliative care or a willingness to enter into such an agreement.
    • Part B Provider certification.
    • Federal DEA license.
    • State DEA license (if required).
    • AHPN - Certification in Hospice and Palliative Care (preferred).
