- Provide direct palliative care consultations in various settings, dedicating focused time to patient care.
- Ensure the standardization of processes and protocols, actively contributing to a network-wide palliative care harmonization workgroup.
- Collaborate with advanced practice and registered nurses, social workers, and administrative team members to coordinate medical care with community physicians.
- Contribute to the expansion of a collaborative program, demonstrating a commitment to clinical excellence and innovation.
- Partner with physician leaders to develop education plans, create curricula, and present on topics such as serious illness communication and advanced illness management.
- Contribute to the establishment of a palliative care fellowship program.
