Job Title: Licensed Practical Nurse (LPN) - PRN

A key member of the Northpoint team, the Licensed Practical Nurse (LPN) provides and oversees medical services to patients. They act as the medical treatment plan coordinator for the multi-disciplinary team, developing and implementing master treatment plans and discharge plans.

Essential Responsibilities:

Interact with patients, observe behaviors, and communicate significant observations to the multidisciplinary team.
Administer medications, educate patients, and utilize de-escalation skills to maintain personal safety.
Provide admission, orientation, education, and discharge plans to patients and families.
Consult with the multidisciplinary treatment team in developing treatment plans and discharge planning.
Maintain a clean, orderly, and safe environment, reporting needs for repairs and maintenance.
Admit and discharge patients, obtain vital signs, monitor symptoms, and document patient records.
Comply with laws, regulations, and scope of licensure, monitoring patients to assess needs and facilitate medical treatment.

Qualifications/Requirements:

Current, unencumbered active license to practice as a Licensed Practical Nurse in the state where services are rendered.
Must be at least 21 years old, hold current nationally accredited CPR/First Aid/AED certification, and have knowledge of computer and electronic medical records systems.
Two years of experience as a Registered Nurse with one year inpatient/residential experience is preferred.
