Job summary
The Care Coordinator is responsible for providing comprehensive care management and care coordination for patients particularly those with serious complex or chronic health problems or psychosocial issues.Responsibilities
- Collaborate and partner with care team to identify patient treatment goals and barriers to care.
- Educate patients on treatments procedures medications continuing care needs connect patients to relevant community resources with goal of enhancing patient health wellbeing increasing patient satisfaction assist patients in navigating care plans e.g request interpreters as appropriate resources for social determinants of health such as transportation services facilitate patient access appropriate medical specialty providers process referrals highrisk populations ensure followup needed document pertinent care management information e.g plans interventions goals assist data collection generation reports participate case conferences periodic meetings scheduled participate quality improvement activities assigned follow up patients assigned group population schedule appointments according clinical scheduling guidelines empanelment provide referrals resources appropriate assist navigate within TrueCare connect inquire general directed provide access coordinate care need hospital ER followups engage them in directed prepare mail letters directed
