    Job Description Job Description:
    Reviews diagnostic and therapeutic medical records for completeness and accuracy. Organizes, manages, and analyzes medical records to ensure quality, security, accessibility, and compliance with accreditation and legal requirements.

    Monitors facility compliance with respect to confidentiality of resident information and complies with HIPAA policies and procedures.

    Assists facility staff with resolving problems of medical records maintenance, establishing acceptable terminology in all related documentation.


    Provides technical assistance, training, and support to facility and clinical staff on health information management requirements, including diagnostic and procedural coding, to meet local, State, and federal guidelines.

    Provides assistance to clinical staff on health records policies and procedures and entering proper data into health records.

    Participates in data collection and presentation for clinical and administrative review.


    Reviews resident medical records to ensure required components are in place such as clinical progress notes, laboratory reports, and treatment records.

    Identifies missing information and informs the appropriate discipline responsible for the documentation of discrepancies.

    Coordinates processes required for Medicaid funding and ensures proper clinical and medical documentation is complete and accurate.


    Corresponds with residents and resident families, and other entities regarding health record information in a confidential manner following all HIPAA policies and procedures.

    Maintains databases and tracks all resident health information disclosures and ensures timely completion of requests.

    Tracks, investigates, and reports privacy issues and breaches of Protected Health Information. Implements legal and regulatory policies, procedures, and requirements for access and disclosure of Protected Health Information.

    Ensures corrective action of internal and external audit findings are completed.
