- Responsible for case mix while focusing on meeting or surpassing budget expectations.
- Assesses and evaluates residents for Medicare Part A coverage pre and post admission.
- Schedules screenings and determinations of coverage for potential Medicare Part A qualifiers.
- Completes and maintains Minimum Data Set (MDS) compliance as per CMS guidelines.
- Coordinates scheduling for annual, quarterly, and significant change MDS as well as timely submissions to government agencies.
- Performs medical record reviews as requested by CMS/HMO's based on their guidelines.
- Oversees/assists nursing staff to ensure MDS compliance.
- Trains and keeps staff updated on MDS and care planning.
- Develops and revises assigned policies and procedures; Ensures nursing department compliance with Medicare, Medicaid, and other insurance regulations.
- Monitors and guides assessment completions in a timely manner.
- Involvement and supervision of MDS assessments inputting, transmission to the state, and validates receipt.
- Schedules and facilitates care plan conferences, and monthly QM meetings.
- Updates and revises patient care plans as necessary.
- Must have a Nursing Degree from an accredited college or university.
- Hold a current active RN license in NYS.
- Minimum of 2 years' experience in Long Term Care as an MDS Coordinator.
- Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
- Excellent written and verbal communication skills with critical thinking abilities.
- Solid understanding of federal, state, and local standards and regulations.
- Ability to organize and prioritize efficiently.
