POSITION SUMMARY: Incollaboration with the patient/family, physicians andinterdisciplinary team, the Case Management Coordinator ensurespatient progression through the continuum of care in an efficientand cost effective manner. The Case Management Coordinator isresponsible for identifying, initiating and managing optimalpatient flow/throughput to enhance continuity of care, smooth andsafe transitions, patient satisfaction, patient safety and lengthof stay management. Support and expertise are provided throughcomprehensive assessment, planning, implementation and overallevaluation of individual patient needs. Care coordination, anddischarge planning are accountabilities of this role. Payer-basedrequests regarding individual members are fulfilled, whereappropriate and pertinent. The Case Manager adheres to departmentaland organizational goals, objectives, standards of performance andpolicies and procedures, continually assuring quality patient careand regulatory compliance.

EDUCATION:

Graduate of an accredited school of nursing, BSN preferredOR Masters Degreein Social Work (MSW) required

CERTIFICATION/LICENSES:

Licensed as a Registered Nurseto practice nursing in the state of New Mexico. Meets contact hourrequirements for licensure, including all state required courses.OR Currentlicensure as an LMSW/LISW/LCSW in New Mexico. Case Managementcertification highly desirable -- ACM (Accredited Case Manager),CCM (Certified Case Manager, RN-BC (Board Certified in CaseManagement)

EXPERIENCE:

Two years' experience in HealthCare setting preferred, with at least one year in an Acute CareSetting.

NATURE OFSUPERVISION:

Responsible to: Director ofCase Management

ENVIRONMENT:

General office environment andgeneral hospital department/nursing units (100% of day).

Blood borne Pathogens: A

PHYSICALREQUIREMENTS:

Ability to movearound the hospital to all units/department for about 80% of theday, in office 20% of the day. Requires close work, good vision,dexterity to write as well as to use computer keyboard.
