    Responsibilities

    ESSENTIAL FUNCTIONS:

    • Perform social history assessment of the patient through observation and patient/family interview, gathering information on the patient's needs, skills, interests, and limitations for the development and execution of the treatment plan.
    • Organize and design programs and activities to meet the social and emotional needs of the patient.
    • Support recovery, symptom alleviation, and the attainment of the highest level of social functioning.
    • Offer crisis management and family sessions to help the family comprehend the effects and complexities of a life situation and its influence on lifestyle.
    • Aid in designing effective treatment plans that apply therapeutic principles appropriately while ensuring patient safety at all times.
    • Engage in meetings with a multidisciplinary treatment team to talk about patient assessment, treatment plan, and ongoing discharge plan.
    • Communicate with family members or help them in comprehending and backing the patient.
    • Handle a caseload and take charge of patient chart documentation.
    • Provide education, process groups, support groups, and referrals as needed.
    • Exemplify a positive, compassionate, and professional demeanor towards customers consistently. When patient needs are not fulfilled, acknowledge and strive to resolve complaints. Acknowledge that patient safety is of utmost importance.

    OTHER FUNCTIONS:

    • Perform other functions and tasks as assigned.
    Qualifications

    EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

    • Bachelor's degree in Social Work from an accredited college or university, with new graduate obligatory.
    • One or more years of experience working with patient population is desirable.

    LICENSES/DESIGNATIONS/CERTIFICATIONS:

    • Current Ohio Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), and Licensed Social Worker (LSW).
    • Current CPR and CPI certifications are mandatory but will be provided upon hiring.
    • First aid might be necessary depending on state or facility regulations.
