    RN - Registered Nurse Case Manager


    Job Summary:
    Constellation Health Services is seeking a compassionate and experienced Home Health Registered Nurse (RN) to join our team.

    The Registered Nurse Case Manager is responsible for providing skilled nursing care and the oversight of care provided by home health aide staff.

    The RN is responsible for the coordination of all patient care services. The Registered Nurse Case Manager supports the philosophy, goals and culture of Constellation.


    Responsibilities:


    Provides skilled nursing care including administration of medications and treatments as prescribed in physician's plan of care for patients in their home setting.

    Coordinates plan of care with all disciplines associated with patient's needs, including family and primary care givers.

    Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever change occurs or at least every 60 days.

    Attends all required staff meetings to enhance professional development.
    Maintains and processes all documentation in compliance with agency standards.

    Will utilize computer based documentation system and submit all required clinical documentation, time records and other required information in required time frame.

    Instructs, orients and supervises other clinical personnel as necessary.
    Develops a plan of care with instructions for home health aide services.
    Provides input to Home Health Aide Supervisor as needed for the purpose of home health aide evaluation.


    Qualifications:
    Graduate of an accredited RN program.

    The employee has the required licensure, education, training and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state.

    Previous home care experience is desirable.

    Bachelor's degree in nursing (BSN) preferred.

    Oasis experience is strongly desired.
    Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care is a plus.
    Have a valid driver's license, reliable transportation and be willing to cover an assigned territory.
    Experience with EMRs and utilizing a tablet and/or computer proficiently.
    Ability to work independently with minimal supervision.
