    **Exciting Opportunity for a Registered Nurse Case Manager**

    **Job Description:**

    **Responsibilities:**

    - **Active Patient Management**
    - Collaborate with medical providers and interdisciplinary teams to set individualized healthcare goals.
    - Utilize problem-solving and critical thinking skills to meet patient and provider needs effectively.

    - **Facilitate Patient Referrals**
    - Work closely with clinical staff to coordinate and track patient referrals.
    - Ensure all necessary information is available for providers, and provide status reports on referrals.

    - **Clinical Contact & Care Coordination**
    - Serve as the primary clinical contact for patients, proactively addressing delays and obstacles to care.

    - **Nursing Duties**
    - Perform various nursing roles, including assessment, documentation, education, and medication administration.

    - **Maintain Competencies**
    - Ensure annual competencies in safety and infection control are met.
    - Promote a clean and safe working environment and adhere to infection control policies.

    - **Communication & Teamwork**
    - Build rapport with team members and participate in department and clinic improvement efforts.

    - **Additional Duties**
    - Assist with special projects to provide comprehensive health services.

    **Position Qualifications:**

    - **Education:**
    - Graduate of an accredited nursing school (Bachelor's degree preferred)
    - High school diploma or GED

    - **Experience:**
    - Minimum of 1 year of case management experience

    - **Licensure & Certification:**
    - Valid, unrestricted RN license in the State of Alaska
    - Basic Life Support (BLS) certification

    - **Knowledge, Skills & Abilities:**
    - Understanding of the nursing process and customer service principles
    - Strong skills in assessment and effective communication
    - Ability to multi-task, work independently, and thrive in a fast-paced environment
    - Team player with a willingness to teach and seek assistance when needed
