    The care manager will ensure all healthcare providers involved are up to date with case information and collaborate with patients' families, friends, and caretakers to effectively share information.

    The care manager will continually develop a network of quality providers and keep accurate patient records. You will link patients to community programs and entitlements, such as housing services and self-management programs.

    The care manager is also responsible for ensuring all documentation, process and procedures are reviewed, completed, and closed accurately for each case.

    This role is under the direction of the Senior Care Manager and Operations Manager.

    Responsibilities for Care Manager
    ·

    Conduct comprehensive assessment during 1st visit to understand patient's current capacity, family history, and determine route of care.
    ·

    Schedule regular meetings with all caregivers, doctors and specialists involved in a patient's care
    ·


    Initiating Services, Access appropriate resource(s), and Consults with patients and their family (NOK), and initiates required services: GN, or POA (prof.

    and non-prof.)
    ·

    Communicate with responsible parties and advocate for the patient/medical facility
    ·

    Identify conflicts in patient care and make judgement calls as to the best path forward
    ·

    Ensure all involved are up to date on the latest tests results and information
    ·

    Maintain an information data stream on multiple patients
    ·

    Oversee the implementation of a patient's medical plan
    ·

    Identify problems ahead of time and initiate appropriate actions
    ·

    Keep up to date and detailed records on all patients
    ·

    Coordinate with other Care Manager to best fall in line with budget
    ·

    Handle of duties assigned by the Senior Care Manager or Operations Manager
    Qualifications for Care Manager
    ·

    Bachelor's degree in social sciences, social work, nursing, or a related field (High School Diploma or equivalent with 5 years of relatable experience)
    ·

    Valid LP Licenses, preferred.
    ·

    Ability to multitask while assisting numerous patients
    ·

    Very strong written and verbal communication skills
    ·

    Excellent analytical and problem-solving abilities
    ·

    A high drive to be successful in all endeavors
    ·

    Ability to work outside of normal business hours
    Possess a calm and comfortable bedside manner
