Job summary
The Home Health and Hospice Team Lead resolves department issues,aware of organizational developments and initiatives and implements as needed for their team.
In addition to the lead responsibilities,maintains clinical competencies and provides professional care to patients of all ages in response to agency needs.
Maintains performance related information and is part of annual reviews.
Mentors and coaches employees;provides performance feedback.Listens to staff needs and concernsand provides resolutions.
Fulfills other agency responsibilities including but not limited to; implementing daily activities
of the office maintaining regulatory compliance reviewing documentation ICD-10 coding monitoring documents re-certifications
performance improvement quality initiatives ensures Joint Commission Medicare third party payer guidelines are followed.
