Job Description

• Provides direct patient care and supervises COTAs in the home setting.
• Coordinates care with the multidisciplinary team and acts as the patient's liaison with other healthcare providers.
• Directs, supervises, evaluates, and provides occupational therapy services to patients in the home.
• Plans and conducts OT programs to restore function and help patients achieve independence.
• Acts as a case manager for postacute services in the home setting, including supervision of home health aides.

• Must maintain a current state driver's license and auto insurance.
• 1year experience as a licensed OT in a clinical care setting or home health preferred.
• Basic Life Support for Healthcare Provider (BLS HCP) from AHA is required.

• Graduation from an accredited school of Occupational Therapy is required.
• Licensed by the Examining Committee to practice Occupational Therapy in the state required.
