We're building a world of health around every individual — shaping a more connected convenient and compassionate health experience At CVS Health you'll be surrounded by passionate colleagues who care deeply innovate with purpose hold ourselves accountable and prioritize safety and quality in everything we do Join us and be part of something bigger – helping to simplify healthcare one person one family and one community at time Registered Nurse – RN-Health Coach Consultant (Hybrid) Location Winston-Salem NC OR within a 30-mile radius Schedule Full-Time (40 hours/week) | Remote/Hybrid We are seeking an experienced Registered Nurse (RN) with strong background in Medical-Surgical nursing to provide Chronic Condition Education This role combines remote work occasional onsite responsibilities for dedicated client in Winston-Salem NC or within 30-mile radius Key Highlights Hybrid Role Primarily remote with potential onsite work up to five days per week as needed Coaching Focus Individual telephonic coaching (1:1) group coaching sessions Language Skills Spanish-speaking RN preferred Work Schedule Onsite Monday–Friday EST flexibility may be required Remote Standard business hours supporting EST plus One evening shift per week late Friday rotation approximately every weeks No weekends required Role Overview The Health Coach Consultant uses collaborative process of assessment planning implementation evaluation to engage educate members promoting informed decisions that support optimal health outcomes Responsibilities Member Assessment & Planning Assess members using clinical tools data review conduct comprehensive evaluations needs benefit plan eligibility integrated programs/services Utilize assessment techniques determine literacy technology capabilities readiness change Develop monitor individualized care plans collaboration members providers meet health goals Enhancing Medical Appropriateness & Quality Care Apply interpret clinical guidelines policy regulatory standards ensure appropriate utilization services Use holistic approach consult supervisors Medical Directors others overcome barriers care Identify escalate quality-care issues established channels Member Engagement Coaching Utilize motivational interviewing influencing skills maximize member engagement promote lifestyle behavior changes Provide coaching education support empower members make informed healthealthcare decisions adopt healthy behaviors Deliver up-to-date information facilitate understanding status treatment options Encourage active participation decision-making providers Monitoring Evaluation Documentation Track progress toward adjust care plans compliance internal policies regulatory accreditation guidelines Required Qualifications Active unrestricted North Carolina compact RN license obtain additional state licenses needed Minimum years clinical RN experience adult medical-surgical nursing High-speed internet access hardwired connection Proficiency Microsoft Office Word Outlook Excel Teams PowerPoint Experience electronic medical records EMR documentation Strong communication skills public speaking Ability travel onsite Winston-Salem as needed Preferred Qualifications Telephonic chronic condition coaching experience Fluency Spanish Virtual group coaching webcam Managed care experience Familiarity Epic EMR functionality Education Associate Degree Nursing ADN required Bachelor Science Nursing BSN preferred Anticipated Weekly Hours Time Type Pay Range The typical pay range this role:
