    Medical group is currently seeking a Primary Care NP/ PA to join our primary care team.

    Group provides comprehensive services for the communities of Mendocino and Lake Counties that include primary care, dental, behavioral health, women's services, and specialty care services.

    There is strong team support and a multidisciplinary care team approach to care. The mission of Health Centers is to provide the highest quality healthcare for everyone in our communities. We ensure that there are no closed doors for those seeking care in Lake and Mendocino Counties.

    Group Offers:

    Salary range of $140,000 - $164,000

    We run a Pod model for our medical team.

    This consists of a group of dedicated Medical Providers, MAs, LVN case managers, and LCSWs to provide true wrap-around care.

    We also have language immersive Pods to provide culturally competent healthcare to our pts,

    You can live on a picturesque lake or in the mountains and still be 5-10 minutes away from the practice and a short drive to major cities.

    Our clinics are located in rural Northern California, 2 hrs. north of San Francisco, 2 hrs. north-west of Sacramento, and 1.5 hr. from the Pacific Ocean.
