We are seeking a Flexible NP or PA to join our team at Home Base Medical Care in Whatcom County, WA.
- Perform an initial comprehensive assessment on all newly enrolled patients and provide ongoing care thereafter.
- Establishes and maintains effective relationships with patients, caregivers and other collaborating physicians/providers when applicable.
