    This is a part time, 20 hour per week position based at Elyria Medical Center.

    Position Summary The Clinical Pharmacist is responsible for the safe, effective, and cost-effective use of medications through the application of specialized knowledge, skills, and professional judgment.

    The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patient's disease conditions, background, need for understanding and participation in drug therapy in a fiscally responsible manner.

    These activities occur in all aspects of pharmacy practice including adult and pediatric inpatient areas, operating room pharmacies, ambulatory oncology and outpatient services (ie., Bolwell Pharmacy).

    The pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific pharmaceuticals pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and/or 4741.

    Essential Functions Accurate order verification Accurate dispensing Minimize order turnaround time Medication monitoring and documentation Documentation of interventions Documentation and performance of medication reconciliation Inventory control and adherence to Hospital Formulary policies Adherence to Hospital and Departmental Policies and Procedures Required For All Jobs Performs other duties as assigned.

    Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.

    Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.


    Qualifications:
    QualificationsEducationBachelor's Degree in Pharmacy Required or Pharm D degree.

    Required Work Experience Previous hospital experience with emphasis on computerized unit dose, sterile products, and patient focused clinical skills Preferred Knowledge, Skills, & Abilities Good communication skills are essential.

    Required Computer and office equipment, automated compounding systems.
