    Description

    This position is for a Licensed Professional Counselor (LPC) to work in the Psych Emergency Access Center, evaluating and transferring psychiatric patients from emergency departments. The ideal candidate will be autonomous, reliable, a critical thinker with a crisis background, have a strong passion for the psychiatric population, and be a self-starter.

    Key Responsibilities:

    • Provides psychosocial assessment and intervention to assist patients and families, including those with clinically complex needs.
    • Collaborates with the multidisciplinary team to ensure comprehensive care.
    • Liaises with community resources to facilitate optimal patient outcomes.
    • Develops and implements individualized plans based on identified psychosocial needs and discharge requirements.
    • Maintains current knowledge of UH network services and promotes their utilization.
    • Serves as a consultant to staff and other disciplines regarding complex psychosocial issues.

    Additional Duties:

    • Performs assigned tasks and duties as required.
    • Complies with all relevant policies and standards.
    • Adheres to the UH Code of Conduct and annual training to maintain Protected Health Information (PHI).

    Requirements:

    • Master's Degree from an accredited Mental Health Counseling program.
    • Hospital or health agency experience preferred.
    • Licensed Professional Counselor (LPC) status required.
    • Excellent verbal and written communication skills.
    • Able to multitask, prioritize, and perform autonomously.
