    Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

    License: Must have the license in hand; pending licenses are not accepted


    Certifications: CPR, BLS


    Must-Have:

    • Proficiency in HomeCare HomeBase (HCHB) EMR system and must have used it within the last 6 months. If used outside of the last 6 months, retraining course during compliance is required.

    Job Description:

    • Responsible for providing, coordinating, and directing the provision of nursing care according to physician's orders based on agency policies and procedures.
    • Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
    • Travel to home visits and periodic office meetings; must have reliable transportation, a current driver's license, and agencyrequired liability insurance.
    • Assigned between 510 patients per day, translating to roughly 68 points per day, with a weekly expectation of 30 points.
    • Handle types of cases including Start of Care, Wound Care, Followup Visits, etc.
    • Open to traveling up to a 75mile radius from the home clinic; mileage is reimbursed at the IRS standard rate.
    • Must be open to oncall rotations; details vary by location and profession.
    • Assigned territory is covering certain city and county areas.
    • Dress code requires scrubs.
    • Must have experience in Home Health; Firsttimers are not accepted.
    • Must be willing to discuss weekend work requirements during the interview process.
    • No contract hours are guaranteed; in case of low patient census, discussions for additional hours at nearby clinics are encouraged.
    • Local candidates are accepted at the same rate.
