    • Dixon, ILPosition TypeFull TimeDescriptionThe Discharge Planner is primarily responsible for managing the development, provision, and monitoring of social services for hospital inpatients and outpatients.
    Responsible for the coordination of in-home services and placement needs for patients and families upon discharge from the hospital. This is an exempt position. Reports directly to the Director, Post-Acute Services.
    EDUCATIONAL REQUIREMENTSBachelor's degree in Social Work or related field required. (May be Social Worker or Registered Nurse)CONTINUING EDUCATION REQUIREMENTSAttends workshops/seminars which pertain to job duties.
    SPECIAL REQUIREMENTSOne year previous social service or related experience preferred. Required to drive personal vehicle in performing some job duties.

    Offered pay rates will be determined by education, experience, and equity with current team members. Range is $24.90 to $37.36
