- Conduct routine monthly visits to SNFs to assess residents' status and assist in goal achievement.
- Attend meetings on behalf of the NP and communicate updates to the Interdisciplinary Care Team (ICT).
- Coordinate care activities within the SNF to ensure resident needs are met.
- Assist with clinical tasks and address gaps in care as needed.
- Engage with residents, families, SNF staff, and other members of the care team.
- Maintain regular communication with the Advanced Practice Clinician (APC) regarding resident status and facility needs.
- Perform resident rounds and participate in Advance Care Planning (ACP) discussions.
- Active RN license in New York.
- BLS certification from the American Heart Association (AHA) or American Red Cross (ARC).
- Minimum of one year of RN experience.
- Strong communication and teamwork skills.
- Ability to prioritize and manage multiple tasks effectively.
