Responsibilities
- Performs patient personal care activities prescribed plan care including hygiene mobility elimination If unable perform task reports RN Director Clinical Services immediately
- Provides maintains safe environment patient Uses equipment safely properly footstools side rails Oxygen etc Practices Universal Infection Control Precautions all contacts patients
- Maintains adequate fluid nutrition intake temperature pulse respiration skin basic elements body functioning changes body functioning record/report appropriately
- Practices safe transfer ambulation normal range motion positioning techniques Identifies need intervention other members home health team ongoing basis reports RN Director Clinical Services respects autonomy right confidentiality patients families Document scare provided response consumer/family submits documentation timely manner Performs within limits preparation experience Ensures compliance local state federal laws Medicare regulations established agency policies procedures Manages caseload delivers cost-effective manner Consistently meets exceeds established service standards Conducts business activities professional ethical manner Uses supplies equipment effectively efficiently Completes minimum hours services year Maintains regular attendance Adheres Code Conduct Interacts staff positive fashion supporting mission vision Complies general orientation agency policies procedures including confidentiality HIPAA guidelines
