    Job Type Full-timeDescriptionThe Care Coordinator at Ascend Medical plays a pivotal role, ensuring comprehensive care between the patient and care team.

    This role involves one-on-one interactions that significantly impact individuals' lives by introducing them to a new approach to primary care, fostering comfort and familiarity.

    The Care Coordinator is responsible for coordinating comprehensive care ensuring the status and completion of referral activity, lab follow-up, prescription orders, refills, vaccine appointment scheduling, and all other aspects of complete care between the patient, physician, specialty office, and primary care office.

    Additionally, the Care Coordinator is responsible for assessing the current processes and workflows, identifying opportunities for improvement, implementing changes as necessary and clinical team development and clinical protocols.


    As Care Coordinator, you will:

    Conduct Patient outreach and follow-up for Ascend patients to ensure adherence to care plan and to determine any other unmet needs.

    Maintain ongoing tracking and appropriate documentation for referrals, lab order, prescriptions (new and refills), vaccine scheduling, and other aspects of the patient's care plan to promote team awareness and ensure patient safety.

    Partner with patients to coordinate scheduling of referrals, provide clinical documentation to the referred specialist, ensure appointments are kept, and schedule PCP follow-up visits thereafter.

    Oversee the completion of best practices by arranging mammograms, GI evaluations for colonoscopies, ensuring smoking cessation and depression screenings are performed prior to visits, collaborating with providers on best practice requirements.

    Partner with Ops & IT departments to develop patient engagement messaging to automate patient outreach where possible. Complete prior authorizations and monitor status of pending authorizations, communicate the determinations to the patient and provider. Ensure patients are kept up-to-date and monitor status of pending authorizations.

    Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).

    Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.

Requirements MA (Certified Medical Assistant) or LPN (Licensed Practical Nurse) with extensive experience and knowledge in Family Practice.

    Salary Description $27-$28 per hour
