    Description

    The Adolescent Wellness Clinic NP/PA serves as the main primary care provider for patients in Skyland Trail's Adolescent intensive services program. This entails pre-intake record review, physical examination for each patient on admission, and ongoing care and counseling for patients as they navigate the program. This position will also interface with the primary psychiatric team, outpatient care team, and the patient's families to communicate medical issues, treatment recommendations, and to coordinate care. Additionally, this employee will participate in treatment team meetings, quality/process improvement, and infection control efforts, in addition to other duties as assigned.

    Responsbilities:
    • Performs initial medical evaluation (H&P), ongoing preventive wellness visits, and other symptom- or disease-focused visits for each patient.
    • Obtains medical history and provides diagnoses, evidence-based treatments, and follow-up recommendations.
    • Appropriately consults with and informs supervising physicians with necessary medical information.
    • Communicates status updates and care plans to patient, team, outpatient providers, and parents, as appropriate. Coordinates care with each patient's outpatient providers at or prior to the point of discharge.
    • Develops service protocols and policies in partnership with Chief Medical Officer and Wellness Clinic Manager.
    • Safely prescribes medication based on efficiency and cost as legally authorized in the state of Georgia. Must be able to select and prescribe correct dosages, routes, and frequencies of medications based on individual patient characteristics.
    • Counsels patients regarding drug regimens, side effects and interactions with food and other drugs.
    • Participates in treatment team meetings in order to communicate patients' health issues and concerns, and to coordinate care with other interdisciplinary staff.
    • Completes documentation of all services provided within 24 hours of the service delivery date using appropriate codes and billing procedures in the electronic health record.
    • Participates in staff wellness initiatives, as needed.
    • Administers medications as needed according to the requirements outlined in organizational policies and procedures.
    • Practices safety, environmental, and/or infection control procedures according to policy.
    • Participates in performance improvement, safety, infection control, and other committees/processes, as needed.
    • Performs other job-related duties as assigned.

    Requirements:
    • Ability to maintain quality, safety, and/or infection control standards
