    DESCRIPTION:


    The Behavioral Health Care Manager (BHCM) works collaboratively within an interdisciplinary care team to improve the health and well-being of patients enrolled in the Care Management Team.

    The BHCM will provide brief behavioral therapeutic interventions and linkages to community resources as well as provide education on chronic medical and behavioral health conditions.

    Utilizing evidence based approaches, the BHCM will promote ongoing engagement with patients and work collaboratively with practitioners across the health care spectrum to provide integrated case management services to moderate to high risk patients.


    Please note:
    If an individual is pursuing licensure as an LCSW, they may be eligible to receive pre-licensed hours and supervision as an ACSW

    Schedule:
    5 days per week, 8 hours per day, Monday - Friday


    ESSENTIAL FUNCTIONS AND DELIVERABLES:
    Provide intensive case management for a set caseload of patients with complex behavioral and medical needs.

    Manage both acute and chronic behavioral/mental health diseases; Reevaluate psychosocial plan of care and ensure continued appropriateness based on the patient's changing needs and condition.

    Conduct initial screening assessments. Conduct ongoing behavioral health and psychosocial assessments. Provide recommendations based on assessment of patients.
    Monitor for warning signs of decompensation and crisis, and respond accordingly.
    Work cohesively with interdisciplinary care team as well as health care practitioners across the continuum.
    Actively participate in the outreach and engagement of new patients. Promote program to patients, families and staff in order to build patient base.
    Meet daily, weekly and/or monthly productivity expectations set by management.
    Communicate, support and encourage patient activation toward achievement of health goals. Utilize evidence based approaches to promote collaboration between care team and patient.
    Prepare and present systematic case reviews in collaboration with interdisciplinary team.
    Assist interdisciplinary team with identifying resources as needed by patient. Provide linkage to community resources.
    Participate in the development of program guidelines and workflows.
    Facilitate linkage to behavioral health clinicians, groups and community-based recreational opportunities.
    Ability to compile and analyze data. Utilize data to provide measurement based care. Monitor and promote effective utilization of healthcare resources through EMR and clinical measures. Track medical and behavioral health outcomes of patients.
    Maintain working knowledge of specialized case and utilization management methodologies and practices; apply concepts to every-day practices.

    Ability to manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail.

    Ensure the implementation and compliance of patient care as per provider's order.

    Work with patients to identify health/wellness goals and incorporate these goals into a shared care plan that facilitates communication among patients and providers.

    Provide Patients, Family, and Caregiver Individual Health Education on Disease Process. Elements of the health education process include verbal and written hand-outs for their reference. Will be required to do patient home visits as necessary.

    Ability to collaborate, teach, support and function in a teaching organization that builds, coaches and embraces interns and residency programs.

    Must demonstrate a passion for the team and have the ability to consistently work with rotating doctors, residents and interns, while continuously building a trusting, safe, patient centered workplace.

    Demonstrate the ability to be culturally sensitive and respect diversity, work effectively and with individuals of different cultures and socioeconomic status.

    QUALIFICATIONS:

    Education:
    A Master's Degree in Behavioral Health Sciences or related field is required

    Licensure/Certification:
    Must have a valid CA Driver's License and Automobile Insurance and BLS or CPR card

    Experience:

    Minimum of one (1) year experience providing case management and mental health services in a community setting with homeless and/or at risk population.

    As a requirement of this position, must complete and pass Epic training and Care Director Training. Must be proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook).Must be able to use widely support internet browsers.
    Must have the ability to use variations of electronic health records and other various databases.
    Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
