- Assists patients with personal care needs such as bathing, feeding, personal hygiene, elimination needs, dressing/undressing, changing linens, turning and repositioning, etc. essential to the patient's health in accordance with the patient's individualized care plan.
- Measures and records patient's temperature, pulse, respiration, blood pressure, intake, and urine output as directed by the plan of care.
- Responds to patient call lights in a timely manner.
- Participates in communication with hospice house staff, including volunteers as needed to complete patient plan of care.
- Observes the patient's physical, mental, emotional, and spiritual conditions and reports any changes to nursing staff in a timely manner.
- Assists the patient with Passive Range of Motion (PROM) when requested and/or per the plan of care.
- Documents services provided timely and accurately in accordance with agency policy and personal care plan.
- Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
- Serves a population newborn through geriatric.

- BLS – BLS is required within three months of hire
- Valid State of Kansas driver's license
