This Behavioral Health Case Manager position supports adults and/or children and families with or at risk of developing serious emotional mental health substance use disorders.Responsibilities
- Manages and monitors assigned client caseload using interpersonal skills that support independence autonomy safety stability.
- Assists clients to link coordinate with health social service support e.g medical hospital psychiatric housing food clothing financial assistance education employment legal social security tribal organizations community services.
