- Evaluate, screen, and treat facility patients/residents in need of Occupational Therapy services.
- Review medical charts to determine the need for services and the potential benefit from skilled services.
- Conduct patient evaluations as per physician orders and develop treatment plans based on patient needs.
- Implement treatment plans through direct care and educate treatment staff, including therapists and other team members.
- Supervise therapy assistants as required.
- Prioritize professional growth by participating in continuing education programs and other learning opportunities.
- Maintain confidentiality of medical records, facility information, and rehab data.
- Ensure accurate billing information following facility protocols on a daily basis.
- Participate in interdisciplinary meetings to share clinical findings and provide recommendations to the rehab team.
- Attend and contribute to family meetings when necessary.
- Adhere to the documentation requirements of the rehab department and facility.
- Provide training and education to staff and families through formal and informal in-services and consultations.
- Follow the policies and procedures established by the facility and Phyt, Inc.

Position Summary: The Staff Occupational Therapist takes on part or full-time responsibilities in an assigned facility. This role involves evaluating patients and providing direct care. Additional administrative tasks might be assigned by the facility administrator or Rehabilitation director.
Qualifications: The Occupational therapist must hold a Bachelors and/or Master's Degree in Occupational Therapist from an Accredited Program. They should possess a current state license, excellent oral and written communication skills, demonstrate competency in therapy treatment and evaluation principles, techniques, materials, and equipment usage, and have knowledge of state and federal accreditation.
Responsibilities:
